      How to use an Into-Sana certificate

      1. The certificate is valid for up to 18 months. The expiration date is indicated on the card.
      2. If the amount of the check exceeds the face value of the certificate, you must pay the difference, but if the amount of the check is less than the face value, the balance is not returned.
      3. It is not possible to split the face value of the certificate into several different payments.
      4. The validity of the certificate extends to services with discounts, offers and services with special prices.
      5. The maximum number of certificates in one check is 1 piece.
      6. To receive medical services based on a certificate, you must first make an appointment for a visit to the relevant Into-Sana medical center by calling the contact center 044-393-0000 (Kyiv), 0482-307-500 (Odesa).

      8. The certificate is not refundable or exchangeable.
      9. The certificate cannot be restored in case of loss.
